CONTEST RULES: Contest is limited to patients of the practice. Employees, their spouses and families are not eligible. There is no limit to the number of entries you may mail in. Winner must be willing to allow their story to be printed in a local newspaper, along with a picture of their smiling face (during a professional shoot. You will also receive an 8x10 glossy and sheet of wallet sized photos).
